6. Duration of appointment (beginning and ending date)

7. Conditions for reappointment

8. Definition of conditions of student's responsibilities and performance criteria (departmental expectations of satisfactory progress towards the degree)

9. Authority that determines whether student is making satisfactory progress (PI, advisor, DGP, Department Head, etc)
